Since the 1960s, the age structure of the Brazilian population has undergone profound transformations, characterized by progressive aging and a consequent increase in the demand for public services, particularly in the areas of health care and social security. This demographic shift imposes the need for substantial adaptations within the health system, which faces an epidemiological transition from a profile dominated by infectious diseases to one increasingly marked by non-communicable chronic diseases (NCDs). Among these, Diabetes Mellitus (DM) stands out due to its rising prevalence and its significant impact on population morbidity and mortality. It is crucial to highlight that the prevalence of DM, its complications, and the associated mortality are profoundly modulated by the intersectionality of race/color and gender. In Brazil, structural racism—manifested through precarious living, housing, and working conditions—systematically disadvantages the Black population, resulting in poorer health indicators compared to the White population. Moreover, gender exerts a significant influence on health behaviors: women generally exhibit a greater propensity to seek medical services, adhere to treatments, and adopt preventive practices, whereas men tend to delay care and neglect symptoms, contributing to later diagnoses and worse clinical outcomes. These differences, combined with racial inequalities, further deepen disparities in the experience and outcomes of DM-related health.In this context, the objective of this study is to analyze the impact of DM on healthy life expectancy (HLE) among adults aged 45 years and older in the states of the Northeast region of Brazil, considering the intersectionality of race/color and gender. This is a cross-sectional panel study, in which an indicator will be developed to simultaneously integrate variations in mortality and health levels.Mortality levels will be obtained from life tables disaggregated by race/color and gender for the years 2013 and 2019, estimated based on the general population life tables provided by the Brazilian Institute of Geography and Statistics (IBGE). DM prevalences will be extracted from the microdata of the National Health Survey (PNS) for the corresponding years. Healthy life expectancy will be estimated using the Sullivan method, which adjusts life expectancy by incorporating the prevalence of specific health states for each age group. The method will consist of applying the DM prevalences obtained from the PNS to the respective risk exposures derived from the life tables, generating the proportion of years lived in different health conditions. This procedure will allow the decomposition of total life expectancy into years lived with and without DM, stratified by race/color and gender.
