Maternal mortality is a significant public health issue, with high maternal mortality ratios (MMR) in low- and middle-income countries, particularly among racialized women. Health inequities have been exacerbated by the COVID-19 pandemic, as the measures necessary to mitigate disease transmission have hindered access to healthcare services during the pregnancy-puerperal period, increasing the risk of adverse outcomes. In Brazil, higher MMRs have been identified in the North and Northeast regions and among Black women. Given these circumstances, this study aims to analyze the impact of the COVID-19 pandemic on maternal mortality in Brazil, focusing on regional and national disparities, from January 2017 to December 2022. To achieve this, an ecological time-trend study will be conducted, examining the bimonthly MMR pattern before (January 2017 to February 2020) and after (March 2020 to December 2022) the first COVID-19 case was diagnosed in Brazil. The maternal mortality ratio will be stratified by geographical region, federal unit, and race/color (Black and White). Maternal mortality data will be sourced from the Ministry of Health's monitoring panel, which includes corrected records post-investigation, while live birth data will be extracted from the Live Births Information System (SINASC/DataSUS). To assess the pandemic's effect on MMR, an interrupted time series analysis (level and slope change) will be employed using a Quasi-Poisson model, accounting for the presence of seasonality and autocorrelation of residuals. In interrupted time series analysis, the event whose influence is being assessed is referred to as the intervention; in this study, the intervention is the COVID-19 pandemic, and the outcome consists of the bimonthly MMRs from January 2017 to December 2022.
