SUMMARY:

Pulmonary rehabilitation (PR) is an intervention aimed at the comprehensive
care of individuals with chronic respiratory diseases. Patients with chronic
obstructive pulmonary disease (COPD) and asthma exhibit low levels of
physical fitness because they avoid physical exercise out of fear of triggering
recurrent symptoms. Wearable devices have been incorporated into
behavioral modification interventions for physical activity in PR protocols.
Objective: To analyze the literature to identify the effects of using wearable
devices to monitor physical activity in pulmonary rehabilitation programs for
chronic respiratory diseases. Methods and analyses: This is a systematic

review, registered in PROSPERO with the code CRD42024504137, for
registration and publication purposes. Searches were conducted in the
electronic databases Medline, Cochrane Central Register of Controlled Trials
(CENTRAL), Embase, CINAHL, and PEDro, as well as in the grey literature.
Baseline data from randomized clinical trials reporting the use of wearable
devices to monitor physical activity in pulmonary rehabilitation programs for
chronic respiratory diseases were included. Articles unavailable in full text and
studies that only discussed the development of algorithms or applications for
disease assessment were excluded. Results: A total of 3940 studies were
found in the selected databases. After the exclusion of duplicates, 1902
articles were analyzed by title and abstract. Of these, 23 were evaluated in full
text and 6 were included according to the eligibility criteria. Additionally, 3
studies were identified through manual reference searches and 1 article was
found through Google Scholar. In the end, 10 articles were selected to be part
of this review. Currently, the research is in the data extraction phase using a
standardized form. The assessment of uncertainty around the evidence will
be conducted using the GRADE system. Final considerations: With this
study, the authors intend to provide a systematic review that offers a
comprehensive analysis of the effectiveness and impact of these devices in
this specific context. This will mainly contribute to the assessment of clinical
efficacy, technological integration in rehabilitation programs, improvement in
exercise adherence, synthesis of evidence, and identification of gaps and
future research needs.
