SUMMARY:

Introduction: Patellar tendinopathy is a common musculoskeletal dysfunction in athletes and is considered an important cause of physical disability. It reduces physical performance and, therefore, represents an important cause for sports retirement. Thus, this condition negatively affects the quality of life of young athletes and is one of the reasons why athletes decrease their levels of physical activity over time. The development of patellar tendinopathy appears to be related to knee extensor weakness; however, other kinetic chain factors such as gluteal weakness, reduced dorsiflexion range of motion, and landing mechanics also appear to be associated with patellar tendinopathy. The most used treatment interventions for this condition, such as the Heavy Slow Resistance Training, focus only on offering progressive load to the patellar tendon. This approach, however, neglects other factors that can directly influence the origin and/or perpetuation of the condition. Purpose: To compare strength, flexibility and jump landing mechanics between athletes with patellar tendinopathy and healthy controls; and to compare an intervention protocol that addresses kinetic chain factors with the Heavy Slow Resistance Training in terms of pain, function, and severity of symptoms in athletes with patellar tendinopathy. Method: This thesis comprises two studies, an observational cross-sectional study and a protocol of a randomized controlled clinical trial. The cross-sectional study will involve 32 athletes, divided into two groups: Patellar Tendinopathy Group (GTP; n=16) and Control Group (GC; n=16) who will be evaluated regarding the strength of hip and knee extensors, knee flexors and knee flexors. ankle plantar flexors using an isometric dynamometer; dorsiflexion range of motion using an inclinometer; and jump landing mechanics using the Landing Error Scoring System (LESS). Results will be analyzed using t tests or the Mann-Whitney test and Pearson or Spearman tests will be used to verify correlations. The clinical trial protocol will involve 28 male recreational athletes, divided into two groups: Kinetic Chain Group (GCC; n

= 14) and Heavy Slow Resistance Training Group (GHSR; n = 14). Both interventions will be performed 3 times a week, for 12 weeks, in a supervised manner. Pain outcomes will be evaluated using a visual analogue scale and disability will be measured with the Victorian Institute of Sport Assessment- Patella (VISA-P) questionnaire, before interventions, after 6 weeks of intervention, immediately after interventions, and 6 months after the interventions. The effects of interventions will be compared using general linear models with a significance level of 5%.
