DEVELOPMENT AND CROSS-VALIDATION OF PREDICTIVE EQUATIONS
FOR FAT-FREE MASS IN SUBJECTS WITH OVERWEIGHT AND OBESITY,
BY BIOELECTRICAL IMPEDANCE ANALYSIS

SUMMARY:
Overweight or obese people have higher risk factors for the development of
cardiometabolic diseases. However, recent evidence suggest that different
phenotypes of obesity exist, each one associated with different degree of
cardiovascular risk. Considering the limitations and scarcity of specific diagnostic
instruments in this context, the objectives of the present study were: 1) to propose
cutoff points for anthropometric indices with biochemical markers for screening
cardiometabolic risk factors in overweight people; 2) develop and validate new
equations to estimate Fat Free Mass (FFM) by bioimpedancemetry for the same
population. For objective 1, the sample consisted of 330 subjects (138 women),
and for objective 2, 305 people were divided into two groups: development
(n=206) and cross-validation (n=99). The anthropometric and biochemical indices
evaluated were VAI, CMI and LAP, the standard body composition technique
used was DEXA and Electric Impendance (BIA) equipment was tetrapolar
monofrequency. To determine cutoff points, the ROC curve was created using
Forward regression, χ² test and the Nagelkerke method. To develop and validate
the equations, the Paired t-test, multiple regression and Bland-Altman plot were
used. Women had the following cutoff points: VAI>4.11 (AUC=0.894), LAP>34
(AUC=0.951) and CMI>1.02 (AUC=0.912) and for men: VAI>2.75 (AUC= 0.918),
LAP>44.7 (AUC=0.923) and CMI>1.66 (AUC=0.923). The study resulted in two
new equations: 1) for people with overweight/obesity: concordance correlation
coefficient (CCC)=0.971; r2=0.94; standard error of estimate (SEE)=2.69kg;
limits of agreement (LOA)=-4.4 to 4.3; and 2) for people with obesity: CCC=0.965;
r2=0.93; SEE=2.27kg; LOA=-5.5 to 4.7. When comparing the new models and
the standard technique, there was no difference in the FFM value (p>0.05). All
anthropometric indices showed good results and can be used to screen
cardiometabolic risk factors, as well as the derived equations can be valid options
for estimating the FFM of overweight/obese individuals with characteristics like
studied sample.
