EFFICACY OF A NANOSTRUCTURED IBUPROFEN GEL ON IN-OFFICE TOOTH BLEACHING SENSITIVITY: A RANDOMIZED, CONTROLLED, TRIPLE-BLIND CLINICAL TRIAL

SUMMARY:

This study aimed to evaluate the effectiveness of an Ibuprofen nanogel in controlling tooth sensitivity caused by in-office bleaching performed with 35% hydrogen peroxide, and its impact on color change. This study was a triple-blind, split-mouth, randomized clinical trial involving 40 participants, where the patient, operator, and evaluator were blinded to the group allocation. Two bleaching sessions were performed, with a 50-minute application of 35% hydrogen peroxide (AutoMixx Plus®, FGM) during each session, and a one-week interval between sessions. The patient received the bleaching treatment on both hemi-arches, but the Ibuprofen nanogel was applied only on one of them, generating the groups Ibuprofen (I) and Placebo (P). Sensitivity was evaluated daily for 14 days using the Visual Analog Scale (VAS) (0 – 10 cm). The VAS data were used to calculate absolute risk, relative risk, and sensitivity intensity. The color change was subjectively assessed using the Vitapan Classical® shade guide and objectively through the EasyShade® spectrophotometer. The objective analysis was used to calculate ΔE, ΔE00, ΔWID, and ΔSGU. Statistical analysis was performed using Jamovi software with paired T-test for color change, Wilcoxon test for bleaching sensitivity intensity, and McNemar test for bleaching sensitivity risk. No statistical difference was found in absolute risk (80 – I; 77 – P) or immediate sensitivity intensity (0.2 – I; 0.3 – P), at 1 hour (0.6 – I; 0.8 – P), 24 hours (0.2 – I; 0.1 – P), or 48 hours (0.0 – I; 0.0 – P). Sensitivity intensity was low to moderate. No statistical difference was found for color change, with ΔE (12.81 ± 4.68 – I; 12.65 ± 5.32 – P), ΔE00 (8.50 ± 3.71 – I; 8.52 ± 4.03 – P), ΔWID (8.06 ± 7.55 – I; 8.34 ± 6.90 – P), and ΔSGU (7.42 ± 2.94 – I; 7.42 ± 2.94 – P) (p > 0.05). The topical application of a nano-structured Ibuprofen gel was not effective in managing postbleaching sensitivity. The protocol also did not interfere with color change.
