Vestibular dysfunction and postural changes in the pediatric population with congenital cytomegalovirus: a systematic review

SUMMARY:

Congenital cytomegalovirus (cCMV) is considered the most common etiology of congenital infection and its main sequela is sensorineural hearing loss (SNHL). Considering the prevalence of SNHL in this population and the anatomical proximity of the cochlear and vestibular organs in the inner ear, vestibular disorders may also occur in children affected by cCMV. This research aims to verify the presence of vestibular alterations and postural balance disorders in the pediatric population with cCMV. This is a systematic review conducted and reported according to PRISMA 2020 and registered in PROSPERO under registration CRD42024549387. Electronic searches were performed in the databases PubMed/MEDLINE, Scopus, Web of Science, Cochrane Library, EMBASE, and Latin American and Caribbean Literature in Health Sciences (LILACS), as well as the gray literature Google Scholar and ProQuest Dissertations & Theses. There were no restrictions regarding language or year of publication. The following inclusion criteria were used: studies conducted in children aged 0 to 12 years diagnosed with congenital cytomegalovirus, performed through urine or saliva samples in the first 21 days of life, neonatal blood samples, serological tests or molecular tests; articles that addressed the assessment of vestibular function and body balance, and that compared symptomatic and asymptomatic groups. In addition, these studies should use validated tests to determine vestibular and/or balance alterations. Only articles from randomized clinical trials and observational studies were selected. The search in the databases resulted in 2,840 studies, of which 923 were removed because they were duplicates, resulting in 1,653 articles to be screened. In Phase 1, after reading the titles and abstracts, 1,630 studies were excluded. Thus, 23 articles were selected for full reading in Phase 2, with 12 articles being removed, totaling eleven studies for the qualitative synthesis. After reading the selected articles, a table will be created with the main information found. This study is expected to find sufficient and validated scientific evidence to support the standardization of the exams used in this population.
