SUMMARY:

Introduction: Health education supported by educational technologies is increasingly recognized as an effective strategy for improving population health outcomes, particularly in early childhood. When implemented in school settings, developmentally appropriate educational resources can enhance health literacy and preventive behaviors at scale. Hearing health promotion is especially relevant given the global burden of childhood hearing loss: the World Health Organization estimates that approximately 34 million children worldwide live with disabling hearing loss, much of which is preventable or manageable with early action. Untreated hearing loss in childhood is associated with adverse effects on language acquisition, cognitive and socio-emotional development, and educational participation. Despite advances in early detection policies, preventive educational initiatives targeting preschool children remain limited. Early childhood education settings represent a strategic and underused opportunity for universal hearing health promotion, particularly through teacher-mediated and technology-supported interventions. In Brazil, although child health promotion is a public policy priority, there remains a shortage of validated, age-appropriate tools developed specifically for hearing health education in preschool populations. Objectives: (1) To develop a multimedia educational object targeting children aged 3 to under 6 years to promote hearing health in the school setting. (2) To map the educational technologies used in health promotion for preschool-aged children (aged 0–6 years) and identify the impacts or repercussions associated with their use across different contexts. Methods: To address the first objective, a methodological study was conducted focusing on the development of an educational object consisting of a short animated video on childhood hearing care for children aged 3 to 6 years in the school setting. The study also included the development of an implementation guide for teachers to support the pedagogical use of the video in the classroom. Prior to implementation with the target audience, the material was evaluated by experienced classroom teachers regarding content clarity, pedagogical adequacy, school applicability, expected impact on children, and suggestions for improvement. The intervention was subsequently implemented across five preschools, involving 24 classes, 352 children, and 19 teachers. Evaluation data were collected electronically. Teachers completed three questionnaires addressing content validation, knowledge and attitudes, and satisfaction. Data were analyzed using descriptive quantitative methods. A minimum Content Validity Index (CVI) cutoff of 0.78 was adopted to assess inter-rater agreement among teachers. Percent agreement (PA) was calculated by multiplying the CVI by 100. To address the second objective, a scoping review protocol was developed following the methodological guidelines of the Joanna Briggs Institute (JBI) for scoping reviews and the Preferred Reporting Items for Systematic Reviews and Meta-Analyses extension for Scoping Reviews (PRISMA-ScR). Results: The Health Content Validation Instrument indicated that the educational object achieved a borderline overall Content Validity Index (CVI = 0.77), suggesting general adequacy with the need for targeted refinements. Domain-specific analysis yielded CVIs of 0.66 for Objectives, 0.85 for Structure/Presentation, and 0.74 for Relevance. Teachers demonstrated good awareness of the importance of hearing health; however, they reported limited practical confidence in addressing the topic. Overall satisfaction with the implementation was predominantly positive, particularly regarding children's learning and engagement. The main barriers identified were insufficient prior teacher training, the need for specialist support, and the need to optimize the video's duration. The scoping review protocol has been developed and will be registered in the Open Science Framework (OSF): https://osf.io/. Conclusion: The findings indicate the need to refine the educational material based on teachers' feedback in order to enhance its attractiveness for children. Furthermore, although the intervention was perceived as relevant and well structured, its effectiveness appears to depend on teachers' prior training, reinforcing the need to strengthen teacher preparation — preferably before the implementation of educational objects for preschool children. The scoping review will enable the systematic mapping of digital educational technologies used in health promotion for preschool children, thereby supporting the refinement of the developed educational object and informing future assessments of its impact on knowledge, engagement, and health behaviors in this population.
