Introduction: Maternal eating habits directly influence their children's complementary feeding. Studies reinforce the idea that maternal food consumption acts as a modulator of the quality and variety of foods offered to children. Prematurity results in conditions that make the introduction of new foods to children even more difficult. Inadequate maternal eating habits associated with issues inherent to prematurity can lead the child to develop an undesirable eating pattern. Objective: To analyze the influence of maternal food consumption on the introduction of food to premature infants. Methodology: This is a cross-sectional, observational, and analytical study involving premature infants treated at the follow-up clinic of the Januário Cicco Maternity School and their mothers. Personal and nutritional data will be collected from premature infants, as well as information regarding the nine indicators proposed in the 2021 technical guide published by the World Health Organization. The maternal variables collected will include food consumption, monthly family income, number of people in the household, maternal education, maternal marital status, maternal employment outside the home, and participation in an income transfer program. Analyses with p < 0.05 will be considered significant. Results: Eighty-three mother-infant pairs were included; 22% of the infants were introduced to food early (before 6 months of corrected age). Regarding the quality of the children's food introduction, 66.2% consumed some type of unhealthy food, however, 92% of the children consumed fruits or vegetables. Most mothers recorded the presence of sugary drinks and reported consuming foods classified as unhealthy. We also observed a high prevalence of inadequate intake of essential micronutrients, such as vitamins D, E, A, and B6, calcium, and selenium. Furthermore, we identified an association between the consumption of unhealthy foods by the mother and child (p = 0.028) and between the consumption of fruits and vegetables by the mother and child (p = 0.004). Furthermore, logistic regression demonstrated that maternal education level and consumption of unhealthy foods by the mother were predictors of the non-consumption of unhealthy foods by the child (p = 0.046). Final considerations: We observed an association between maternal dietary practices and those of their prematurely born babies, with a particular emphasis on the impact of maternal education. The high inadequacy of maternal intake of essential micronutrients is concerning, considering the potential impact of maternal nutrition on the child's nutritional health, both through breastfeeding and through its role in shaping eating habits. Thus, the results reinforce the importance of food and nutrition education strategies aimed at mothers, focusing not only on the qualitative adequacy of children's diets but also on promoting healthy food choices within the family environment.
