EDUCATIONAL AND CARE TECHNOLOGY: CONSTRUCTION AND VALIDATION OF AN INDIVIDUALIZED PLAN AS A STRATEGY FOR QUALIFYING SPECIALIZED AMBULATORY CARE AND TEACHING-SERVICE INTEGRATION.

Addressing chronic degenerative conditions is a critical challenge for public health in Brazil, requiring a holistic and coordinated approach. Care for chronic degenerative conditions is a priority in the Brazilian Unified Health System (SUS) due to the high demand for care and the need for early interventions to prevent complications.The objective of this study is to develop and validate an Individualized Care Plan, configured as educational and care technology to strengthen teaching-service integration. Based on a systematic review of the literature, an instrument will be developed which, at a later stage, will undergo a content validation process with the participation of specialized judges. After content validation, the instrument will proceed to the appearance validation stage. The final product of this work will optimize the sharing of patient care between Specialized Outpatient Care (AAE) and Primary Health Care (PHC) services. This intersectoral coordination aims to ensure continuity and comprehensiveness of patient care, promoting more efficient and humanized care. The implementation of effective communication and the joint development of care plans seek to optimize resource allocation and increase the resolution capacity of health services. It is hoped that these results will promote a positive transformation in the care of patients with chronic conditions, benefiting not only individuals but also the efficiency and sustainability of the health system as a whole.
